We want to help you live healthier right here in our community. Whether you are newly diagnosed or need an update, there is always something to learn. Participants are encouraged to bring family members, friends or caregivers. At Memorial Hospital we offer several programs to help you manage your diabetes.
Thrive with Diabetes is an accredited program with group workshops or individual sessions. Our staff includes a Registered Dietitian/Nutritionist, Certified Diabetes Educator, Pharmacist, Nurse Diabetes Educator and an ACSM Certified Clinical Exercise Specialist. Our group program covers eating, activity, monitoring, medications, problem solving, reducing risks, goal setting and more. New groups start quarterly and usually meet for six consecutive weeks - able to join at any time during the program cycle. Individual sessions focus on your issues and needs. Your schedule and insurance policy determines your best fit.
Medical Nutrition Therapy or nutrition counseling is 1:1 with the dietitian. Evening and weekend appointments are available. Again coverage varies based on insurance provider policies.
Diabetic Support Group meets monthly at different locations in Chester. This group emphasizes FOOD, FRIENDS & FUN. Topics are health related, but not always diabetes specific. Click for our current newsletter.
Conversation Map Training will begin in January 2017. Though not as in-depth as the accredited program, four group sessions cover most aspects of diabetes. Maps are set-up like a board game, with questions drawn from a card deck. Participants’ responses to the questions guide the session. While these classes provide an option for those without insurance coverage, they also complement our accredited group or individual program sessions.
