How to Enroll

Cardiac and pulmonary rehabilitation services are usually covered by Medicare and most insurance plans for the conditions listed below. A physician's prescription is required to enroll in our rehab programs. Our staff will work with you and your physician to determine the most appropriate plan for your individual needs. La Medicina Nuclear es una modalidad dentro de las imágenes de diagnóstico, que analiza la fisiología del cuerpo en lugar de la anatomía. Los servicios del Departamento de Medicina Nuclear del Memorial incluyen, entre otros, estudios cardíacos, estudios óseos, estudios de la vesícula biliar y estudios de infecciones.

Las imágenes se obtienen utilizando cantidades muy pequeñas de materiales radiactivos, comparables a la radiación con una radiografía de diagnóstico, administrados por vía oral, intravenosa o debajo de la piel. El radionúclido viajará a través del torrente sanguíneo hasta el órgano objetivo. Luego, se escanea el órgano en busca de estructuras anatómicas, función y enfermedad. Los procedimientos de medicina nuclear pueden usarse para diagnosticar hipertiroidismo (enfermedad de Grave), estrés cardíaco, problemas ortopédicos, función anormal o bloqueos de la vesícula biliar o el hígado, así como para evaluaciones metabólicas del cerebro y la estadificación de pacientes oncológicos.

Individuals who have had a serious cardiac condition, as listed below, can benefit from cardiac rehab. 

  • Heart attack (in the past 12 months) 

  • Heart bypass surgery 

  • Stable angina or stable chest pain  

  • Stent placement, angioplasty 

  • Heart valve replacement or repair 

  • Heart transplant 

  • And other heart conditions 

Talk to your provider about whether your condition qualifies. 

Following a cardiac condition, your caregiver or provider will recommend actions you can take to improve your recovery. After discharge, managing these actions on your own can be challenging. In a cardiac rehabilitation program, individuals have a support team to help them remain accountable to improve their physical fitness, minimize cardiac symptoms and reduce their risk of heart problems in the future. 

Pulmonary
Rehabilitation
Program

Individuals who have chronic lung disease which causes shortness of breath can benefit from pulmonary rehab. The program helps increase activity tolerance and improve strength, endurance, and flexibility. The program may benefit people with the following conditions:

  • Chronic obstructive lung disease (COPD)

  • Persistent post COVID-19 symptoms

  • Asthma

  • Bronchiectasis

  • Cystic fibrosis

  • Emphysema

  • Lung transplant

  • Pulmonary fibrosis

  • An exposure to inhalants that may have caused chronic lung symptoms

During the pulmonary rehab program individuals receive education and a customized exercise program to help manage their lung disease and feel better by breathing easier to improve your quality of life.

Program Goals

The goal of cardiac and or pulmonary rehabilitation is to improve a patient's quality of life by way of exercise, medication management, risk reduction and training to self-manage daily life consistent with the cardiac or pulmonary disease process. 

 

Why Should I Attend? 

Research has consistently shown that patients who complete rehabilitation for their heart or lung conditions have better outcomes including: 

  • Decreased breathlessness 

  • Improved ability to perform every day tasks 

  • Better quality of life 

  • Better understanding of their health condition(s) 

  • Reduced need to return to the hospital because of better condition management 

What to Expect

Following discharge from a hospital, or a chronic heart or lung condition diagnosis, a physician may provide a referral to a cardiac or pulmonary rehabilitation program to help improve a patient’s health and recovery. During the evaluation at Memorial Hospital’s Cardiac Pulmonary Rehab Center our team develops a customized plan to personalize the exercise program to fit the individual’s needs. Each session includes educational instruction and supervised exercises. Sessions last 60-90 minutes (about 1 and a half hours) and take place multiple times a week. The number of sessions will depend on the individual’s medical benefits plan which our staff pre-authorize.

During the sessions patients will exercise in a safe environment with skilled staff and will be surrounded by others who have had similar health experiences and are on their paths to wellness, creating a supportive environment.

Upon completion of the program at Memorial Hospital’s Cardiac Pulmonary Rehab Center, graduates will be able to continue to make these lifestyle changes as part of their everyday life continuing their program exercise independently at the full functioning gym at Memorial Hospital’s Therapy & Sports Rehab Center located in Chester, IL. (Monthly facility access fees will apply.)

Our team of rehab staff work as personal trainers and educators to individualize a wellness plan to meet everyone's unique needs. They empower participants with knowledge, purpose, and desire to improve their health and fitness within safe levels. Information and education will also be provided to help achieve goals such as: 

  • Weight loss

  • Smoking cessation

  • Stress management

  • Healthy cholesterol levels

  • Lower blood pressure

  • Healthy eating

  • Diabetes Management

  • Medication Management
