Memorial's Swing Bed Program Memorial Hospital’s Swing Bed program provides a wide range of interdisciplinary rehabilitation services to patients with short term rehabilitation and medical needs. P atients may be referred by physicians, acute care hospitals or other health care professional, for admission to swing bed status. Our swing bed team will meet with the patient to evaluate their condition and level of function. The team will assess the patient’s needs, plan treatment and establish an estimated time frame to meet the team’s goals. Family members are an integral part of the treatment team. Dr. Monica Lukose is a new family practice provider that will be joining Memorial Hospital's Rural Health Clinic teams in Chester and Steeleville, Illnois. She will begin seeing patients in August of 2024. BACK TO ALL PROVIDERS Dr. Monica Lukose, M.D. Family Medicine Dr. Monica Anil Lukose received her medical doctorate from Ross University School of Medicine in Barbados and is currently completing family medicine residency training at Mount Sinai Health System in Chicago. Dr. Lukose’s training at Mount Sinai has allowed her to gain experience in a variety of patient care settings of different acuity and complexity, both acute and chronic and including level one trauma. Dr. Lukose, originally from Vancouver, Canada, moved to the United States to complete her family medicine residency and remained in the U.S. after meeting her husband - Varun Varghese. As a resident at Mount Sinai Hospital- Family Medicine program, Dr. Lukose was awarded the Manthani J. Reddy, M.D. award for Resident of the Year in 2022. She was promoted to chief resident during her third year of residency. She is a member of the American Academy of Family Physicians and looks forward to board certification this Spring. Dr. Lukose’s training and experience has given her the opportunity to provide care to patients of all ages as well as those of various social and economic backgrounds. She has interests in both inpatient and outpatient family medicine and enjoys women’s health. Dr. Lukose takes a preventative approach in treating her patients, and values connecting and building relationships with them. Before you view the pricing information for any hospital, we wanted to provide you with some basic background information about: The provided pricing information. How it relates to a final hospital bill or pricing information included in an explanation of benefits from your health insurer. Understanding Pricing for Healthcare Services Before you view the pricing information for any hospital, we wanted to provide you with some basic background information about: The provided pricing information How it relates to a final hospital bill or pricing information included in an explanation of benefits from your health insurer While this information should help you compare prices from hospital to hospital, it is unlikely that your final bill will exactly match the prices listed. We’ll explain more about that later. Each hospital sets a “gross charge” for every individual service rendered to patients within their "chargemaster" or CDM (charge description master). These gross charges do not include any discounts that may be offered, and they serve as the starting point from which payment is negotiated with individual insurance payers for specific insurance plans. As a patient receives services throughout their visit, a charge for each service provided is generated on their account, resulting in a claim that is submitted to the patient’s insurer. You should know that patients will almost never pay the listed gross charge for healthcare services. However, under federal law, all insurers, including Medicare and Medicaid, must be billed the amount listed on the chargemaster for those services. These charges are rarely paid in full due to the contracted payment rates negotiated between hospitals and insurers. The EOB provided by individual insurers details each patient’s actual cost for services provided. Hospitals typically cannot publicly disclose the negotiated rates, making it difficult to compare final prices from one hospital to the next using the gross charge information from the chargemaster. The Information You Will See: Standard Charges & Shoppable Items Standard Charges Medicare has defined several different types of standard charges that should be available for patients to see. They are: Gross charges Discounted cash price Payer-specific negotiatedcharge De-identified minimum negotiated charge De-identified maximum negotiated charge Here’s a quick overview of each. Gross Charges The gross charge is the full list price from the hospital chargemaster. Gross charges can vary, sometimes greatly, from hospital to hospital for the same procedure or service based on how each hospital manages its charges and costs. Charges can vary based on geography, physician supply and medication preferences, the kinds of services the facility typically provides, and the expertise required to deliver these services. External factors also play a role: The cost of living in a given area can have a significant effect on wages, which is a major factor in cost calculations for hospitals. Drug and supply costs also vary greatly depending on which (if any) group purchasing organization the hospital is part of. Discounted Cash Price The second type of standard charge defined by Medicare is called a discounted cash price, which is the price offered to patients willing to pay in cash at the time of service without involving insurers. Payer-Specific Negotiated Charge The payer-specific negotiated charge is the charge that a hospital has negotiated with a third-party payer for an item or service; this is sometimes referred to as the "allowed amount" on an EOB. This charge amount will likely vary from payer to payer and even between insurance plans for the same insurance payer. De-identified Minimum Negotiated Charge The de-identified minimum negotiated charge is simply the lowest charge that a hospital has negotiated across all insurers for an item or service. De-identified Maximum Negotiated Charge The de-identified maximum negotiated charge is the highest charge that a hospital has negotiated with all insurers for an item or service. Shoppable Services The high degree of variation in charging practices and differences in reimbursement methodologies between insurance payers make it difficult for patients to get the intended full-benefit of “pricing transparency.” Medicare wanted to give patients another way to compare prices, so they’ve also asked hospitals to create a list of shoppable services. Medicare defines "shoppable services" as a service that typically can be scheduled by a patient in advance on a non-urgent basis. Medicare has identified 70 shoppable services that all hospitals should include and has asked hospitals to each choose at least 230 additional shoppable services that they perform most frequently. Every shoppable service will contain an easy-to- understand description of the item or service and standard charges information, including the gross charge, discounted cash price, average negotiated charge for each insurance payer, de-identified minimum charge, and de-identified maximum charge. You'll also see a specification for whether the procedure is done on an inpatient or outpatient basis, as procedures done on an inpatient basis may incur additional charges, such as room and board charges View Pricing Information Where Can You Get More Information? Calling your insurer is always a good idea if you're considering an elective procedure and want to get a general idea of your out-of-pocket costs. Your insurer can help you understand how your coverages and deductibles work as well as your current payment history as an essential first step. If you want to get a more general idea of costs or compare your likely costs to those incurred by others, there are a few public pricing resources to consider: The Centers for Medicare & Medicaid Services Medicare releases annual payment information for inpatient and outpatient procedures. All-Payer Claims Database Some states have large-scale databases that collect medical, pharmacy, and dental claims, as well as eligibility and provider files from private and public payers. Diabetic & Nutrition Resources For lifestyle change to be lasting, you have to focus on where you are now. Change is a process. What you want has to be what you are truly willing to do. As you master one skill, then another one follows. While information is important, being able to apply that information to your life is crucial. One-on-one visits can get you started. However, group programs provide the longer term support to make change a habit. Diabetes Management Individual sessions available. There are many ways to improve diabetes control. Body Balance coaches you towards sustainable change with the support of others in similar situations. Learn facts to create habits that stick. Cardiac Pulmonary Rehab Center How to Enroll Cardiac and pulmonary rehabilitation services are usually covered by Medicare and most insurance plans for the conditions listed below. A physician's prescription is required to enroll in our rehab programs. Our staff will work with you and your physician to determine the most appropriate plan for your individual needs. To learn more, contact the Cardiac and Pulmonary Rehab Center at 618-826-4581 ext. 1151. Memorial Hospital’s Cardiac Pulmonary Rehab Center will be opening May 1, 2024. Rehabilitation programs at the center will provide comprehensive care to patients with heart and lung conditions. Through the program, individuals are given a structured program that includes education, counseling, and exercise to strengthen their lungs and heart. Participants in the program have access to various tools and a full support system to help them recover. The new rehabilitation center is on the lower level of the main hospital. The state-of-the-art facility features top-of-the-line exercise and weight training equipment with space for group sessions. Under the supervision of Dr. John Groll, Cardiologist, each patron has a complete team of professionals - trained registered nursing staff, a dietitian nutritionist, respiratory therapist, pharmacist, and social worker - supervising them through the program and helping them to reach their recovery goals. The professional rehab staff monitor all exercise sessions to ensure safety and progress. Cardiac Rehabilitation Program Individuals who have had a serious cardiac condition, as listed below, can benefit from cardiac rehab. Heart attack (in the past 12 months) Heart bypass surgery Stable angina or stable chest pain Stent placement, angioplasty Heart valve replacement or repair Heart transplant And other heart conditions Talk to your provider about whether your condition qualifies. Following a cardiac condition, your caregiver or provider will recommend actions you can take to improve your recovery. After discharge, managing these actions on your own can be challenging. In a cardiac rehabilitation program, individuals have a support team to help them remain accountable to improve their physical fitness, minimize cardiac symptoms and reduce their risk of heart problems in the future. Pulmonary Rehabilitation Program Individuals who have chronic lung disease which causes shortness of breath can benefit from pulmonary rehab. The program helps increase activity tolerance and improve strength, endurance, and flexibility. The program may benefit people with the following conditions: Chronic obstructive lung disease (COPD) Persistent post COVID-19 symptoms Asthma Bronchiectasis Cystic fibrosis Emphysema Lung transplant Pulmonary fibrosis An exposure to inhalants that may have caused chronic lung symptoms Talk to your provider about whether your condition qualifies. During the pulmonary rehab program individuals receive education and a customized exercise program to help manage their lung disease and feel better by breathing easier to improve your quality of life. Program Goals The goal of cardiac and or pulmonary rehabilitation is to improve a patient's quality of life by way of exercise, medication management, risk reduction and training to self-manage daily life consistent with the cardiac or pulmonary disease process. Why Should I Attend? Research has consistently shown that patients who complete rehabilitation for their heart or lung conditions have better outcomes including: Decreased breathlessness Improved ability to perform every day tasks Better quality of life Better understanding of their health condition(s) Reduced need to return to the hospital because of better condition management What to Expect Following discharge from a hospital, or a chronic heart or lung condition diagnosis, a physician may provide a referral to a cardiac or pulmonary rehabilitation program to help improve a patient’s health and recovery. During the evaluation at Memorial Hospital’s Cardiac Pulmonary Rehab Center our team develops a customized plan to personalize the exercise program to fit the individual’s needs. Each session includes educational instruction and supervised exercises. Sessions last 60-90 minutes (about 1 and a half hours) and take place multiple times a week. The number of sessions will depend on the individual’s medical benefits plan which our staff pre-authorize. During the sessions patients will exercise in a safe environment with skilled staff and will be surrounded by others who have had similar health experiences and are on their paths to wellness, creating a supportive environment. Upon completion of the program at Memorial Hospital’s Cardiac Pulmonary Rehab Center, graduates will be able to continue to make these lifestyle changes as part of their everyday life continuing their program exercise independently at the full functioning gym at Memorial Hospital’s Therapy & Sports Rehab Center located in Chester, IL. (Monthly facility access fees will apply.) Our team of rehab staff work as personal trainers and educators to individualize a wellness plan to meet everyone's unique needs. Medical Explorers Program The Memorial Hospital Organization, MH, is a registered Medical Explorers Post 8618. The program is for high school and college students who are interested in Healthcare Careers. Whether you already know what occupation you would like to pursue or if you have not decided on a specific career field yet, through Medical Explorers you will have opportunities to learn about many careers, participate in hands-on activities and meet healthcare professionals. The MH Medical Explorers program is a 9-month program with one group meeting/event held the third Thursday of each month, September through May. Clinicals, Internships & Job Shadowing Scholarships Scholarships Scholarships are available to qualifying high school seniors, employed students, and Randolph County residents who meet specified criteria and who are selected by a scholarship committee. Medical Explorers Scholarship Memorial Hospital offers a Medical Explorers Scholarship each year for High School Seniors and College Students throughout Randolph County, IL who are members of the Memorial Hospital Medical Explorers Post. Upon completion of the program and submission of a reflection essay a scholarship is awarded by the Memorial Hospital Auxiliary. Auxiliary Nursing Scholarship The Memorial Hospital Auxiliary Nursing Scholarship is designed to provide residents of Randolph County, Illinois financial assistance when pursuing a degree in a field of nursing. The purpose of the scholarship is to promote a nursing career at Chester Memorial Hospital by providing money for tuition and fees to residents of Randolph County. The scholarship award of up to $1,200.00 will be given to any selected resident of Randolph County who will be completing the RN Program of study during the current academic year. The prospective graduate of a LPN Program will qualify for a $500.00 scholarship award. The school attended need not be an Illinois institution; however, it must be accredited or recognized as an approved program by appropriate agencies. The scholarship is awarded based on a written application and interview focusing on character, scholastic achievement, professional promise, and financial need. Tuition Reimbursement Program Tuition reimbursement is available to all regular Full Time and Part Time employees that have served one year of continuous 20+ hours weekly of service. To be eligible for reimbursement, an employee must sign an Agreement of Reimbursement, agreeing to remain employed for one year following any reimbursement under this policy, or otherwise become liable for payment of reimbursement.
