Did one our nurses go above and beyond to show you or your loved one EXTRAORDINARY care? DAISY Nominations Did one our nurses go above and beyond to show you or your loved one EXTRAORDINARY care? Nominate them for a DAISY Award for Extraordinary Nurses ® Nurses at Memorial Hospital and our Rural Health Clinics are being honored with The DAISY Award for Extraordinary Nurses ®. The award is part of the DAISY Foundation’s mission to recognize the extraordinary, compassionate nursing care they provide patients and families every day. The DAISY Foundation is a not-for-profit organization, established in memory of J. Patrick Barnes, by members of his family. Patrick died at the age of 33 in late 1999 from complications of Idiopathic Thrombocytopenic Purpura (ITP), a little known but not uncommon auto-immune disease. (DAISY is an acronym for Diseases Attacking the Immune System.) The care Patrick and his family received from nurses while he was ill inspired this unique means of thanking nurses for making a profound difference in the lives of their patients and patient families. “When Patrick was critically ill, our family experienced first-hand the remarkable skill and care nurses provide patients every day and night. Yet these unsung heroes are seldom recognized for the super-human, extraordinary, compassionate work they do. The kind of work the nurses at Memorial Hospital are called on to do every day epitomizes the purpose of The DAISY Award,” Said Bonnie Barnes, FAAN, President and Co-Founder of The DAISY Foundation. “We are proud to be among the healthcare organizations participating in The DAISY Award program. Nurses are heroes every day. It’s important that our nurses know their work is highly valued, and The DAISY Foundation provides a way for us to do that,” said Chief Nursing Officer, Susan Diddlebock. In addition to the DAISY Award for Extraordinary Nurses, the Foundation expresses gratitude to the nursing profession internationally in over 3,900 healthcare facilities and schools of nursing through the J. Patrick Barnes Grants for Nursing Research and Evidence-Based Practice Projects and for nurses participating in medical missions. More information is available at DAISYfoundation.org Nurses may be nominated by patients, families, and colleagues. Sinus Infection School and/or sports physicals (Payment due prior to exam.) Employment Physicals Convenient Care Hours Sunday-Saturday 8am-8pm How Does Convenient Care Work? A Triage Nurse will perform an initial assessment. The Triage Nurse and/or Physician shall have final authority in determining whether the patient’s medical condition qualifies them for treatment in Convenient Care. If the patient does not qualify , they will be offered Emergency care. If the patient does qualify , they will be charged for this service at a reduced rate. Any testing (lab tests, X-rays, etc.), supplies used, or medications given are at an additional cost. Additional reviews of X-ray or lab work may be necessary and will be charged separately. If patient becomes worse and needs to be treated in the Emergency Department, charges will be billed at the regular Emergency Department rate. In order to do Convenient Care at a reasonable cost, the patient is always asked for the Convenient Care co-pay the day of treatment. As a Convenient Care patient, your treatment becomes secondary to patients in the Emergency Department. It is critical for the staff to treat those with severe conditions first. Why Choose Convenient Care Emergency Room claims can be denied because the insurance company determines them to be “non-emergent”, which leaves the patient responsible for the bill. Convenient Care is billed at a lower rate to your insurance provider or to you. You can be seen after normal clinical hours for illnesses or injuries that do not require emergency care. You can even be seen on weekends. Download Brochure

  • ICAHN 2018 | MHChester

ICAHN recently recognized Memorial Hospital, Chester, for its ongoing commitment to exceptional quality of care, during a ceremony held October 29th at the I-Hotel and Conference Center, Champaign, IL. Memorial Hospital won honors in every available category Memorial Hospital gains ‘4-Star’ patient satisfaction rating, wins honors in all categories of excellence CHESTER, IL – The Illinois Critical Access Hospital Network (ICAHN) recently recognized Memorial Hospital, Chester, for its ongoing commitment to exceptional quality of care, during a ceremony held October 29th at the I-Hotel and Conference Center, Champaign, IL. Memorial Hospital won honors in every available category. As part of the Federal Office of Rural Health Policy Medicare Beneficiary Quality Improvement Program (MBQIP), Memorial Hospital and other critical access hospitals were asked to voluntarily participate in four defined domains of quality outcomes. These four domains include: Patient Safety/Inpatient; Patient Engagement; Care Transitions; and Outpatient Measures. “Often, in rural hospitals, having a devoted staff member to abstract and submit this data can be both time-consuming and burdensome,” said Angie Charlet, ICAHN Director of Quality, Educational Services, and Compliance. “This hospital realizes how important quality of care and an engaged staff is for its patients…From its top leadership on throughout the facility, each strives for continued quality on a daily basis, and that is what we are celebrating today.” Memorial Hospital was recognized for its quality of care outcomes (for participating in all measures in all four domains), for being a top performer (over 95%) over two years in two domains, for being most improved (over one year in any one measure or domain); and for achieving a ‘4-Star’ HCAHPS Rating. The Center for Medicare and Medicaid Services (CMS) created the HCAHPS Star Ratings for consumers to more quickly and easily access the patient experience of care information provided on the Hospital Compare website. Star ratings will also allow consumers to more easily compare hospitals. “We are very proud of our team for achieving this level of excellence in health quality,” said Brett Bollmann, Administrator. “This is truly a team effort that recognizes our dedication to delivering a healthcare experience of which our patients and staff can be proud.” ICAHN is a network of 57 small rural Illinois hospitals dedicated to strengthening the operations of its member hospitals through collaboration. The organization is composed of Illinois hospitals who have earned critical access hospital designation by the Center for Medicare and Medicaid Services. ICAHN is recognized nationally for its work with rural healthcare and administers several state, federal, and private healthcare programs. The ICAHN office is located in Princeton, IL.
