July 16, the Chester Clinic will offer FREE Sports Physicals for ALL Chester School District Students in elementary school and high school participating in 2022-2023 school sports.

July 23 - Steeleville Family Practice: Steeleville, St. Marks & Trico Students Only
Masks will be required as long as Hospital's are still required to follow the IDPH Mask Mandate
Things to bring:
- A Mask - Masks will be required. Your school's sports physical form.
- Signed Form - Parents are asked to fill out their portion and sign the forms prior to the patient being seen. Download a copy of the form at: www.mhchester.com/free-sports-physicals
No appointment is necessary. Free physicals are for grade school or high school students participating in sports for the 2022-2023 school year. NO COLLEGE students will be seen.
