What's Included in a
School Physical
During your child's school physical we do a a head-to-toe check-up that ensures your child is healthy, growing well, and ready for school activities. Their exam includes a review of their health history, basic screenings, and a full exam.
If your child also needs a sports physical, that can be completed with this exam.

Who Needs a
Sports Physical?
Any child that will be playing elementary or high school sports will need a sports physical.
Free Sports Physicals are available from Memorial Hospital in July.
If your child also needs a school physical, their sports physical can be completed at the same time. Tell your provider you also need the sports physical form completed.

Memorial Hospital's Convenient Care
offers walk-in school & sports physicals
Monday–Friday: 8am–5pm
Saturday: 8am–1pm
NOTE: Vaccinations are not offered in the Convenient Care. If a patient receives a school physical in the Convenient Care, and they need vaccinations, the school will not consider the physical complete until the vaccinations are completed.

When to Schedule
Most schools accept physicals completed within the past 12 months, so summer is an ideal time to schedule your child’s appointment. Getting it done early helps avoid the rush and ensures everything is ready before school starts.
