Signed Form - Parents are asked to fill out their portion and sign the forms prior to the patient being seen. Download a copy of the form at: www.mhchester.com/free-sports-physicals
No appointment is necessary. Free physicals are for grade school or high school students participating in sports for the 2026-2027 school year. NO COLLEGE students will be seen.
