Vaccination Information Consumer Drug Take-Back Program In an effort to help reduce the risk of drugs in the home that can potentially harm children, teens or adults, the Pharmacy has set up a secure, safe drop-off location in its retail service area. The box is accessible during operating hours. Accepted: Unused or expired prescription medication (Including Schedule II-V controlled substances. Unused or expired over-the-counter medication. Pet medication. Not Accepted: Schedule I controlled substances. Illegal drugs. Thermometers, inhalers, aerosol cans, needles, lotions/liquids, hydrogen peroxide. The Memorial Community Pharmacy provides prescription and over the counter medication retail services to the public. Located on the West end of the hospital, customers have easy access to services through the side entrance door. There is also reserved parking for customers at this entrance with a Curbside Pick-Up location. Upon arrival, customers can call the number listed on the sign, 618-826-6134, a staff member will deliver your medications directly to your vehicle and help you with payment processing. As an added convenience, established customers, have access to the 24hr Pick-Up Locker located in the ER lobby. This feature allows customers to retrieve their medications during hours that fit their schedule. The electronic screen is quick and easy to use with payment accepted at the kiosk. Customers receive text or email notifications when their prescriptions are ready, they can pay online prior to pick-up (link expires after 24 hours) or with a card during pick-up. Just another way Memorial is improving services to meet your needs. Drive Thru now available at the Memorial Community Pharmacy Customers now have the convenience of accessing their prescriptions through our Drive Thru window, located in the arced drive by the Pharmacy entrance. Our Drive Thru is open during our normal services hours, Monday-Friday 7:00am-5:30pm and Saturday 9:00am-1:00pm. How do I sign-up to receive my prescriptions through Memorial Community Pharmacy? During your doctor's visit , inform your providers that you would like to use Memorial Community Pharmacy. T o move a current maintenance medication to Memorial Community Pharmacy, notify the pharmacy staff by calling 618-826-6134, and they will transfer your prescription(s). New customers can establish care by: Phone - call the pharmacy during business hours at 618-826-6134. Mail/Email - Fill out the Intake Form and return to the pharmacy. If you would like to utilize the 24-hour locker for pick-up, please complete the Patient Consent Form . If you are picking-up your medications via counter pick-up or drive-up , you will need to bring your current prescription card, driver's license or Social Security Number (for eligibility check or picking up of a controlled substance). If you are picking-up your medications via the 24 Hour Locker Kiosk , you will need to have your 6-digit code to verify your identity. If you did not pre-pay for your prescriptions through the link provided in the text/email notification, you will need to bring a debit, credit, or HSA card to complete payment at the kiosk. The kiosk only accepts electronic payment, no cash or checks will be accepted at the kiosk for payment of medications. How do I pick-up my medications from the 24 Hour Locker? Customers wishing to use the 24 Hour Pick-Up locker to claim their prescriptions, will need to fill out an Intake Form as listed above and submit a Patient Consent Form . When you arrive at the hospital, park in the reserved location in the ER Parking Lot. Enter through the ER doors and proceed to the locker located to the right of the admitting desk. Follow the prompts on the screen to verify your identity and claim your medications. If payment is required, you will be instructed to use the card reader below the screen. If you require a consultation with a pharmacist, you can use the handheld receiver to video chat with the pharmacist. Once you have completed your transactions a locker will pop open with your medications inside. Here is a quick tutorial video on how to use the touch screen, with a consult, to claim your medications. Our pharmacy staff is happy to do an in-person walk through of our kiosk to make you more comfortable with using it. Stop by the Pharmacy Monday-Friday 7:00am-5:30pm and someone will be happy to assist you. I am having trouble with the 24 Hour Locker Kiosk. Notify our registration staff in the ER entrance lobby, and they will contact a pharmacy member to assist you. Our ER registration desk is staffed 24/7, so no matter when you are picking up your medications, someone will be there to help you. Where is my code to pick-up my prescription? When you establish care as a patient of the Memorial Community Pharmacy, by filling out the Intake Form and Patient Consent Form , you will be asked to provide information regarding whether or not you would like to use the 24-hour kiosk and how you would like to receive notifications. If you choose text message , you will receive a text whenever your prescriptions are ready. This text will include a 6-digit code to assist in verifying your identity when you arrive at the kiosk to claim your medications. It will also include a link that allows you to submit payment, within 24 hours of receiving the message, to pre-pay for your medications. Payment is also accepted at the kiosk during pick-up. Occasionally a text will be dropped. If you haven't received notification within 2 days of request, contact a member of the pharmacy at 618-826-6134 to receive your pick-up code. If you choose to be notified by email, and did not receive an email notification, please check your Junk/Spam folders to make sure it did not go there. If you did receive an email, you can scan the QR code from your mobile device or printed email, or enter the 6-digit code in the email to claim your medications once you arrive at the kiosk. Sandy Otten of Chester, retired from Memorial Hospital on April 26th after 45 years of service. She was hired in August of 1972 for a position in the Health Information Department (Medical Records), and retired as the Performance Improvement Director. Sandy Otten of Chester, retired from Memorial Hospital on April 26th after 45 years of service. She was hired in August of 1972 for a position in the Health Information Department (Medical Records), and retired as the Performance Improvement Director. When reflecting on the many changes she has witnessed at Memorial over the years, she believes that technology has improved the most. When she began, typewriters and 3-part carbon paper were used, whereas, fax machines, copiers, and computers became standard equipment. She even saw the transition to a new Electronic Health Record system before she retired. Sandy states that she will never forget the friendships that were developed with the girls in HIM that she worked with every day. “It was a privilege and joy to work among the professional, caring, and compassionate staff at Memorial Hospital,” said Sandy. She is looking forward to doing all of those “fun” things with granddaughters and family, a little traveling, and relearning how to play the piano. She thinks she will miss the challenge and excitement of the ever-changing healthcare environment. During her time at Memorial, Sandy was a dedicated employee and her passion for her position could be seen by all of her co-workers. Memorial Hospital thanks Sandy for her outstanding years of service, and her commitment to encourage staff to provide safe, high quality healthcare and service to the patients.

These protocols provide a Stroke patient with “clot busting” medications, in a timely manner, with the hope to reduce the significant residual affects. Illinois developed and passed legislation, in 2009, that allowed large hospitals to demonstrate their ability to provide these evidence based practices and protocols that earned them the distinction of being a Primary Stoke Center. In 2014, Illinois recognized that there was an opportunity for rural or critical access hospitals to work in conjunction with the Primary Stroke Centers to provide evidence based protocols for Stroke patients, including administering “clot busting” medications and transport to a Primary Stroke Center. Memorial Hospital is proud to be able to provide this lifesaving opportunity to the community. As HealthCare Providers, Physicians and staff at the hospital understand that opportunity for the best outcome requires education on Stroke symptoms and reaction, to our Community that we serve. Symptoms of a Stroke include: Sudden numbness or weakness of the face, arm or leg Sudden confusion or trouble speaking or understanding others Sudden trouble seeing in one or both eyes The American Stroke Association provides the simple pneumonic – FAST - in an effort to remind the general public of the signs/symptoms of a Stroke: F- FACE: Ask the person to smile, Does one side droop? A- ARMS: Ask the person to raise both arms. Does one arm drift downward? S- SPEECH: Ask the person to repeat a simple phrase. Is their speech slurred or strange? T- TIME - If you observe any of these signs, call 9-1-1 IMMEDIATELY. The “clot busters” the Hospital gives for a Stroke must be started within 3 hours of the “LAST KNOWN WELL.” By activating EMS or calling 9-1-1, patients and their families are starting a chain reaction. Using an ambulance allows for the hospital staff to alert the Stroke Team and prepare for the patient’s treatment. The goal for treatment is to start the “clot busting” medication in the first 60 minutes of arrival. The program is for high school and college students who are interested in Healthcare Careers. Whether you already know what occupation you would like to pursue or you have not decided on a specific career field yet, through Medical Explorers you will have opportunities to learn about many careers, participate in hands-on activities and meet healthcare professionals. The MH Medical Explorers program is a 9-month program with one group meeting/event held the third Thursday of each month, September through May. Meetings are typically held in the conference room of Memorial Hospital unless otherwise indicated. Student Participation There will be a limited number of students accepted. Applicants will be notified of acceptance the first week of September. Students are required to attend 75% of the Post meetings/events. Students are required to complete 4 hours of volunteer time during the program duration. These hours can be at Memorial Hospital or in the community. More details will be provided at the first meeting. Students are encouraged to participate in Memorial Hospital community events including: Health Fairs, Youth Education Events, Blood Drives, and Parades.
